    摘要: 研究背景:睡眠品質與憂鬱情緒不佳在高齡者中為常見的問題,但在長照機構高齡者較為嚴重,若長期累積下來易導致健康結果受到負面的影響。近年研究指出,雙耳節拍能有效改善睡眠品質與憂鬱情緒問題,但應用在長照機構高齡者睡眠品質與憂鬱情緒實證仍然缺乏。因此,本研究旨在探討雙耳節拍音樂對長照機構高齡者睡眠品質與憂鬱情緒之影響。研究方法:本研究採隨機控制實驗,於2022年11月至2023年3月在彰化某長照機構進行收案。參與者被隨機分配到實驗組聆聽雙耳節拍音樂合併台語老歌或對照組聆聽台語老歌音樂。所有參與者在上午8:30~9:30與下午4:00~5:00進行聆聽,每週6次,每次15分鐘,連續2週。於介入前、後進行匹茲堡睡眠品質量表、短版老人憂鬱量表、專業睡眠手環、心率變異量測。統計方式主要採用Wilcoxon signed-rank test分析兩組介入前、後測組內差異分析;Wilcoxon rank-sum test與Mann-Whitney U-test分析兩組介入前後測變化量組間差異分析。研究結果:本研究共招募14位長照機構高齡者(男性2位、女性12位),實驗組平均年齡81.50±3.93歲,對照組平均年齡83.75±3.58歲。研究結果顯示聆聽雙耳節拍音樂合併台語老歌相較於聆聽台語老歌音樂介入,實驗組在主觀睡眠品質有較明顯改善趨勢,雖尚未顯著(p=0.051)。聆聽台語老歌相較於聆聽雙耳節拍音樂合併台語老歌介入,對照組在短版老人憂鬱量表有顯著降低(p=0.031)。實驗組與對照組組間相比,在效果量結果顯示PSQI之主觀睡眠品質呈大效果量(r=0.521)、睡眠手環之入睡後清醒次數呈中等效果量(r=0.416)與HRV之SDNN呈中等效果量(r=0.448)。研究結論:本研究結果指出,雙耳節拍音樂或許有機會改善長照機構高齡者睡眠品質及緩解憂鬱情緒,但因為研究樣本數少,未來仍需要更多樣本數及相關研究來證實。
    Background: Poor sleep quality and depression symptoms are common problems among the older population, but they become more serious in older adults residing in long-term care facilities. If left unaddressed for extended periods, these issues can lead to negative impacts on health outcomes. Recent studies have suggested that binaural beats can effectively improve sleep quality and depression symptoms. However, there is still a lack of empirical evidence for applying this method to enhance sleep quality and reduce depression symptoms among older individuals in long-term care institutions. Therefore, this study aims to explore the effects of binaural beat music on the sleep quality and depression symptoms of older adults in long-term care facilities.Methods: This study employed a randomized controlled experiment conducted in a long-term care institution in Changhua from November 2022 to March 2023. Participants were randomly assigned to either the experimental group, which listened to binaural beat music, or the control group, which listened to old Taiwanese songs. All participants listened to the assigned music for 15 minutes each time, six times a week, at 8:30-9:30 am and 4:00-5:00 pm, over two consecutive weeks. Before and after the intervention, the Pittsburgh Sleep Quality Index, the Short Version Geriatric Depression Scale, the professional sleep bracelet, and heart rate variability measurements were conducted. The statistical methods mainly involved the Wilcoxon signed-rank test to analyze differences within each group before and after the intervention, as well as the Wilcoxon rank-sum test and Mann-Whitney U-test to compare differences between the experimental and control groups.Results: A total of 14 participants (2 males and 12 females) from long-term care institutions were recruited for this study. The average age of the experimental group was 81.50�3.93 years, while the average age of the control group was 83.75�3.58 years. The results of the study indicated that the binaural beat music intervention showed a tendency to improve the subjective sleep quality of the experimental group, although it was not statistically significant (p=0.051). However, the control group, which listened to Taiwanese old songs, demonstrated a significant reduction in the Short Version Geriatric Depression Scale (p=0.031). Additionally, the control group exhibited a significant reduction in the short version of the depression scale when compared to the group receiving both binaural beat music and Taiwanese old songs (p=0.031). Effect size analysis indicated a large effect size (r=0.521) for the subjective sleep quality measured by PSQI, a moderate effect size (r=0.416) for the number of awakenings after falling asleep measured by the sleep bracelet, and another moderate effect size (r=0.448) for the SDNN of HRV.Conclusion: The results of this study suggest that binaural beat music may have the potential to improve sleep quality and alleviate depression symptoms among older adults in long-term care facilities. However, due to the limited sample size, more research with larger sample sizes is required to confirm these findings in the future.
