    Abstract: 本研究目的為探討退化性膝關節炎老人疼痛與睡眠品質之相關性。採相關性研究設計,針對台中市某地區醫院60歲以上的退化性膝關節炎患者,患者本身之疼痛與睡眠品質現況,立意取樣有效問卷共100份,本研究是以結構式問卷調查法(structured questionnaires)與實際訪談的方式進行資料蒐集,運用之研究工具為結構式問卷、視覺類比量表(Visual Analogue Scale: VAS) 及匹茲堡睡眠品質量表(Pittsburgh Sleep Quality Index;PSQI) 評估。統計分析採套裝軟體進行分析,分別採用描述性統計、t檢定、相關性分析及迴歸分析等統計方式來進行樣本檢測與探討,結果顯示影響睡眠質量的因素為:「本身情緒狀態」(B=0.275,P = 0.005)和「疼痛程度」(B=1.804,P = 0.000)。故疼痛程度對睡眠品質的影響很大。台灣邁向高齡化社會,老年人膝蓋問題,嚴重影響晚年生活,並與中風、失智和冠狀心臟病並列造成老年人失能的前四大疾病。逐步迴歸分析依據本研究結果有助於醫護人員了解退化性膝關節炎老人的疼痛與睡眠品質相關性,護理實務上在臨床照護患者時,須注重個人生理、心理以及社會等層面;護理教育及研究宜提供患者個別化擬訂相關的標準化護理措施,主動評估提供給予預防保健諮詢及衛教單張或手冊,加強患者關節炎之日常活動健康指導,並提供不同復健治療措施的衛教計畫,讓患者能因應疼痛引起的睡眠障礙,進而幫助其減輕疼痛的不適及合宜的醫療照護指引,提升正確治療方針有更優質的睡眠品質。
    The purpose of this study is to probe the correlation between pain and sleep quality in elderly patients with degenerative knee Osteoarthritis. Descriptive correlation research design, for patients with degenerative knee arthritis over 60 years old in a regional hospital in Taichung City, the patient's pain and sleep quality status, a sample of 100 valid questionnaires, the research is structured Data collection was conducted using structured questionnaires and actual interviews. The research tools that we used were structured questionnaires, Visual Analogue Scale (VAS) and Pittsburgh Sleep Quality Index (Pittsburgh Sleep Quality Index; PSQI) Evaluation. Statistical analysis was performed using the software package.Descriptive statistics, t-test, correlation analysis, and regression analysis were used to conduct sample detection and discussion. The results showed that the predictive factors affecting sleep quality were: (B=0.275,P = 0.005) and "degree of pain"(B=1.804,P = 0.000). Therefore, the level of pain affects the quality of sleep influentially.Taiwan is moving towards an aging society, the knee issues of the elderly have seriously affected the life of elderly society, and it is combined with stroke, dementia and coronary heart disease which cause the four major diseases of the elderly. Stepwise regression analysis based on the results of this study will help medical staff to understand the correlation between pain and sleep quality in elderly patients with degenerative knee arthritis.In nursing practices, paying attention to the physical is a must, psychological and social aspects of clinical care patients; nursing education and the study should provide patients with individualized standardized care measures, and proactively evaluate health education programs that provide preventive health advice and health education leaflets or manuals, strengthen daily health guidance for patients with arthritis, and provide different rehabilitation treatments. It can alleviate the sleep disorder caused by pain, and help it to relieve the pain and discomfort and appropriate medical care guidelines, to improve the treatment which will create a better quality of sleep.
