    Abstract: 生物阻抗分析(Bioelectrical impedance analysis, BIA)可快速應用於四肢瘦肉軟組織之測量,本研究針對不同站立式多肢段BIA裝置應用DXA於台灣地區老人進行比較,並探討其精度差異。 以台灣地區可自行行走的年齡大於六十歲以上的老人(男、女分別為30、35人),分別應用Tanita BC418(50 KHz)、Biospace InBody230(20, 100 KHZ)、Tanita MC-780MA(5, 50, 250 KHz),分別簡稱為BC418, InBody230, MC780,以及雙能X光吸收儀(DXA),測量上肢、下肢與四肢的瘦肉軟組織質量(lean soft tissue, LST),並分別以 ordinary least product regression、平均絕對百分比誤差(mean absolute percent error, MAPE)為整體效果指標。 研究結果顯示,65名老人的體脂率為35.3±7.3%,受測者於BC418、InBody230、MC780相對於DXA於四肢(appendicular)的LST(LSTapp)的相關係數(r)與標準估計誤差(standard error of the estimate, SEE)分別為0.813、0.662、0.826與2.13 kg、2.75 kg、2.64 kg,三種BIA裝置與DXA的於平均差異分別為5.655 kg、3.352 kg、4.532 kg,其對應的Limits of agreement(LOA)分別為0.332 to 11.066 kg、-2.953 to 9.744 kg、-0.258 to 9.356 kg, MAPE分別為38.24 – 48.56% 、23.67 – 31.45%與30.67 – 41.34%。 站立式的多肢段BIA裝置用可自行行走能力的老人於四肢瘦肉軟組織質量為便利的測量裝置,但不同的BIA裝置因廠牌型號的不同,其測量的精度亦有不同與限制,應用於臨床評估的需小心判斷應用。
    Lean soft tissue (LST) of limbs is quickly measured by bioelectrical impedance analysis (BIA). This study, compared the accuracy of different multi-segments BIA devices by dual-energy X-ray absorptiometry (DXA) reference in older community-dwelling Taiwan adults.The subjects recruited elderly people over 60 years old (35 females and 30 females) in Taiwan. The study used Tanita BC418MA (BC418), Biospace InBody230 (InBody230), Tanita MC-780MA (MC780), and DXA to measure the LST of upper limbs, lower limbs, and extremities. Ordinary least product regression and mean absolute percent error (MAPE) was taken as the overall effect indicators of the study.In the study group the average percent body fat (PBF%) of 65 elderly people was 35.3±7.3%. The correlation coefficient (r) and standard error of the estimate (SEE) of BC418, InBody230, and MC780 relative to the LST (LSTapp) of DXA in the appendicular were 0.813, 0.662, 0.826, and 2.13 kg, 2.75 kg, 2.64 kg, respectively. The average differences between the three BIA devices and DXA were 5.655 kg, 3.352 kg, and 4.532 kg, respectively. The corresponding limits of agreement (LOA) were 為0.332 to 11.066 kg, -2.953 to 9.744 kg, and -0.258 to 9.356 kg, respectively. The MAPE were 38.24 – 48.56%, 23.67 – 31.45% and 30.67 – 41.34%.The standing multi-segment BIA device is a convenient device for measuring the LST in the limbs of older community-dwelling adults having different measurement accuracy and limitations due to different brands and models. When applied to clinical evaluation, the application needs to be carefully used.
