    Title: 臺灣長照服務採用美國PACE計畫模式可行性之分析

    Abstract: 背景
    臺灣2018年超過65歲以上的老年人口比率已達14%以上,為正式進入高齡社會(Aged Society)。許多研究指出美國PACE (Programs of All-inclusive Care for the Elderly,PACE)的整合照護模式,可以得到有效醫療成本上的控制及良好的照護結果。所謂PACE,係一種全包式老人長期照護計畫,由一個科際整合團隊,評估虛弱老人需要,擬訂計畫,預收固定期間定額給付,來管理與提供整合且連續性的長期支持與服務,給仍失能且需技術性護理的老人。
    目的
    利用文獻查證及文獻回顧與本文作者在2013年衛福部南投醫院實驗的試辦計畫進行綜合性的比較,藉由計畫實證研究專業人員之觀點,以分析臺灣長照服務採用美式PACE計畫的條件與配套措施。
    方法
    收集1980至2019年發表於PubMed、EBSCO、ProQuest資料庫做搜尋,中文文獻部分則是在華藝線上圖書館做搜尋,網站方面是用Google Scholar學術搜尋,進行資料探勘,同時事後檢視當時利用問卷蒐集衛福部南投醫院參與此計畫8位專業人員的觀點,進行深入之分析與探討。
    結果
    美國針對長照的資金籌措與制度永續性方面,部分學者指出州政府可鼓勵私營保險公司認可PACE等綜合護理服務模式,以增加保障管理索賠方面的支付意願。在本土專業人員問卷方面,大多數認為PACE計畫導入至臺灣實施,可能需要整合相關健保與長照資源、培育專職及專業人力、備足經費才具可行性。
    結論與建議
    未來PACE導入臺灣長照服務系統的可行性是有機會的,依據2013年臺灣PACE試辦計畫的經驗顯示,建議未來台灣長照採行PACE模式可行性之配套措施包括:「足夠的經費」、「專職的人力」、「培育專業人員」、「整合相關資源」和「適當的宣導」。
    Background
    In Taiwan, in 2018, the ratio of the elderly population over 65 years old has reached more than 14%, which is officially entered into the Aged Society. Many studies have pointed out that the integrated care model of PACE(Programs of All-inclusive Care for the Elderly) in the United States can achieve effective medical cost control and good care results. The so-called PACE is an all-inclusive long-term care plan for the elderly. It is an interdisciplinary integration team that assesses the needs of the frail elderly, prepares plans, and receives fixed-term fixed payments in advance to manage and provide integrated and continuous long-term support and services. Give elderly people who are still disabled and need technical care.
    Objective
    Using literature review and literature review, the author compares the pilot project of the experiment in the Nanfu Hospital of Weifu in 2013, and analyzes the perspective of the empirical research professionals to analyze the long-serving service in Taiwan using the American PACE program. Conditions and supporting measures.
    Method
    The collection was published in PubMed, EBSCO, and ProQuest databases from 2014 to 2019. The Chinese literature section was searched in the Huayi Online Library. The website was searched by Google Scholar for data exploration, and the questionnaire was used afterwards. We collected the opinions of eight professionals from the Nanfu Hospital of Weifu Department and conducted in-depth analysis and discussion.
    Results
    In terms of fundraising and institutional sustainability of long-term photography in the United States, some scholars have pointed out that state governments can encourage private insurance companies to recognize comprehensive care service models such as PACE to increase their willingness to pay for safeguards management claims. In terms of local professional questionnaires, most people think that the introduction of PACE plans into Taiwan may require the integration of relevant health insurance and long-term resources, the cultivation of full-time and professional manpower, and sufficient funds to be feasible.
    Conclusion and Suggestion
    The feasibility of introducing PACE into Taiwans long-serving service system in the future is an opportunity. According to the experience of the 2013 PACE pilot project in Taiwan, it is suggested that the supporting measures for the future adoption of the PACE model in Taiwan will include: "Sufficient funds" "Full-time manpower", "cultivating professionals", "integrating related resources" and "appropriate advocacy".
