Abstract: | 本研究目的:探討止痛藥物給予時機對於膝關節置換術病人之(1)疼痛控制滿意度(2)麻醉全期舒適度(3)麻醉全期舒適度的差異性。
方法:本研究採橫斷式調查。以65歲以上老年人為研究對象,分配術中給予止痛藥物及術後給予止痛藥物各33人。於術後24小時內收集個案資料。研究工具包含疼痛控制品質問卷(AmericanPainSociety-Patient outcome questionnaire, APS-POQ)、Kolcaba舒適度問卷(Perianesthesia Comfort Questionnaire, PCQ )、手術前後麻醉照顧病人滿意度問卷(PatientSatisfaction with Perioperative Anesthetic Care questionnaire, PSPACq),以面對面訪談問卷,收集資料以進行描述性統計、卡方檢定及羅吉斯回歸分析加以整理。
研究結果:術中給予止痛藥物(1)在單變項羅吉斯回歸疼痛控制滿意度高於術後給予止痛的患者(OR=3.496,p=0.023),再納入多元羅吉斯回歸止痛藥給予時機的不同(OR=1.217,p=0.778),無顯差異。(2)在麻醉全期舒適度與術後給予止痛的患者無差異性(3)在麻醉全期滿意度與術後給予止痛的患者無差異性(OR=1.244,p=0.741)。
結論:建議膝關節置換術的患者,術中給予止痛藥物來緩解患者術後疼痛程度,並提升全期舒適度、麻醉全期滿意度。術前給予患者麻醉相關衛教、術後不適之症狀及止痛相關事項。
The purpose of this study is to explore the impact of analgesic drugs for patients undergoing knee arthroplasty in the areas of (1) satisfaction with pain control (2) the comfortableness during the whole period of anesthesia (3) variations in the comfortableness during the whole period of anesthesia.
Methods: A cross-sectional survey was conducted in this study. Thirty-three elderly people over 65 years old were allocated intraoperative and postoperative analgesics respectively. Case data were collected within 24 hours after operation. The research tool included a questionnaire on the quality of pain control (AmericanPainSociety-Patient outcome questionnaire, APS-POQ) and Kolcaba comfort questionnaire (Perianesthesia Comfort Questionn, PCQ)、Patient satisfaction questionnaire after anesthesia before and after operation (PatientSatisfaction with Perioperative Anesthetic Care questionnaire, PSPACq), data were collected by face-to-face interview and descriptive statistics, chi-square test and logistic regression analysis were used to sort the data.
Results: in single-variable logistic regression, the pain control satisfaction in intraoperative administration of analgesics (1) is higher than postoperative analgesia (OR=3.496, p=0.023), and in multiple logistic regression, there is no significant difference in the timing of administration of analgesics (OR=1.217, p=0.778). (2) There is no difference between the comfort of anesthesia and the postoperative analgesia. (3) There is no difference between the satisfaction of the anesthesia and the postoperative analgesia. (OR=1.244, p=0.741).
Conclusion: it is suggested that patients undergoing knee replacement surgery should be given pain relief to relieve postoperative pain, and to improve the comfort level and satisfaction during anesthesia. Prepare patients for anesthesia related education, postoperative discomfort symptoms, pain relief and related matters before surgery. |
