Abstract: | Objectives: Compare to general public, intellectual disabilities tend to have more obesity and chronic diseases. Among ten leading causes of death in Taiwan 2014, lots of them are related to chronic diseases. And cardiovascular diseases are common death leading causes to intellectual disabilities. Currently, there are few studies explored the chronic diseases and the medical utilization of intellectual disabilities. So this study took hyperlipidemia for an example, to investigate the medical differences of general public and intellectual disabilities.
Methods: In this study, we analyzed the national health insurance database from 2008~2012. First, when the ICD-9 code 317-318X appeared more than 3 times, we saw them as intellectual disability group, and the others are non-intellectual disability group. Then, we used ICD-9 272 more than 3 times or the use of anti-hyperlipidemics (ACT CODE =C10) to screen out patients, we choose patients' age over 18 as our subjects. SAS9.3 statistic software was used to analyze the differences between these two groups.
Results: showed that the prevalence of hyperlipidemia in population over 18 years old in 2008-2012 was 7.7%~10.2%, which general public is 8-10.7% and intellectual disabilities is 3.9-5%. After the logic regression analysis, we found that the prevalence of hyperlipidemia of general public are remarkably higher than intellectual disabilities, but the prevalence of hyperlipidemia of 19-44 years old intellectual disabilities was higher than general public.
About the medical utilization, after statistic test to the number of medical use, we found that the there was no difference between intellectual disabilities and general public. Further analysis showed that in general public, male, Taipei business group, $ 0<X<$18780, common identity and comorbidity 0 points, the number of medical visits is higher than the intellectual disabilities in the same group; and also showed that the general public in Taipei business group, 0<X<$18780, major disabilities, low income, comorbidity 1 points, average medical fee is higher than the intellectual disabilities in the same group.
Conclusion: The study showed that the hyperlipidemia prevalence or medical utilization in general public is higher than the intellectual disabilities. Whether it is caused by health inequity or it is caused by other relevant factors, need further investigation. |
